Born in the port city of Marseille in October 1997, Theo, brother of Lucas and son of former French footballer Jean-François Hernandez, began his career in Majadahonda’s youth academy. The following year he moved to Atletico Madrid’s youth team, making his Tercera Division debut with the club's second team.
He went on loan to Alavés the following season and after a convincing spell in the blue and white shirt, was signed by Real Madrid. His first season with the Galácticos began with a European Super Cup victory and ended with lifting Champions League trophy. He was permanently signed by AC Milan after a one-year loan at Real Sociedad.
He had a promising first season (2019/20) with the Rossoneri, scoring seven goals in 36 appearances, and his second season was even better. In the season that saw AC Milan return to the Champions League, Theo established himself as the best left-back in the league, with seven decisive goals across 33 games (a tally that rises to eight goals in 45 appearances if the Coppa Italia and Europa League are included).
